“While traveling approximately 25 MPH in a 1999, Chevrolet z71 truck left rear firestone tire, size: p265/75r16, DOT: w273wmb189 experienced a blowout. There were 12,000 miles on tire, and it was original equipment. Blowout was approximatley 1 1/2 inches in length, and was in center of the tread.”
