“The contact rented a 2016 U-Haul E450. The contact stated that while in the parking lot and placing the vehicle in drive, the contact tapped on the accelerator pedal, then when the brakes were applied and the vehicle suddenly accelerated. The contact removed her foot from the brake, and it continued to accelerate and crashed into a parked U-Haul truck. There were no reported injuries, fire, airbag deployment, or police report filed. It was unknown if there were any warning lights illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. …”

When attempting to have the brake fluid service performed, it was discovered that the reservoir cap indicated DOT 3 brake fluid was in the vehicle. The owner's manual indicated that the brake fluid for that vehicle was type DOT 4 brake fluid. The vehicle was taken to an independent mechanic, but was not serviced due to the confusion. The contact was concerned that due to the two different types of brake fluid labeling, it could potentially cause a brake failure. …”
