“The front driver-side door lock and opener (actuator) stopped working using the key fob and needs to be manually locked from the inside. When I try to access the door after manually locking driver side and using my key fob for the remaining 3 locks, the unit won't automatically open. This causes a safety risk considering I need to access the driver's side back or front passenger side to open the door. According to what I've read this is a known issue.”
NHTSA complaint #11745416, filed 2026-06“The contact owns a 2021 Toyota Rav4. The contact stated that the front driver’s side door failed to lock securely. The contact stated that the door could not be locked while using the key fob, the keypad, or locked manually. The cause of the failure was not yet determined. The local dealer and manufacturer were not notified of the failure. The failure mileage was 108,000.”
NHTSA complaint #11731485, filed 2026-04, 108,000 miles“Gear shift makes noise when moving it, as in you can actually hear the gears/electrical equipment moving and making noise when going from park to drive. The keyless entry when the fob is near, just doesn't work well from the drivers side... I can stand there trying to allow it to unlock and it just locks again or wont unlock the passenger door even if allowing it extra time to.”
NHTSA complaint #11740401, filed 2026-05“The front passenger door started to not unlock when using the keys or door buttons to unlock it, you would have to manually unlock the passenger door by reaching across from the driver's side of the car. For about a week, that was happening, then that same door started to not lock using the key fob, handle or door buttons, so both locking and unlocking could only be done manually. …”
NHTSA complaint #11574711, filed 2024-02“The contact owns a 2021 Toyota Rav4. The contact stated that after parking the vehicle overnight, the following morning it was discovered that the rear hatchback door had erroneously opened. The contact stated that a severe thunderstorm had occurred over night however, the keys had not been used nor were any buttons depressed to cause the hatchback door to become unlocked. The cause of the failure was not determined. The local dealer and manufacturer were not notified of the failure. The failure mileage was 4,055.”
