“This minivan has the entertainment system, when using the TV screen, it blocks the rearview mirror where you are unable to see out of the back window. I have not seen any warning labels. I am unsure if it has been inspected by the manufacturer, and police and insurance reps have not inspected mine.”
NHTSA complaint #11704150, filed 2025-12“The contact owns a 2026 Toyota Sienna. The contact stated while driving at an undisclosed speed, the rear window glass shattered. No warning lights were illuminated. The vehicle was taken back to the residence. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The contact immediately pulled over and discovered that the rear window had shattered without incident as glass debris had spread all over the truck of the vehicle. The contact was able to flag down a police officer in the area who assisted the contact with the failure. A police report was not filed as no evidence of a crime or nefarious dealing was discovered. …”
NHTSA complaint #11753280, filed 2026-07, 3,000 miles“The factory-installed backup camera on my brand-new 2026 Toyota Sienna XSE has a critically low resolution, resulting in a deeply blurry, fuzzy, and pixelated image. Furthermore, the video feed is heavily letterboxed into a small rectangle in the middle of the large 12.3-inch screen. This poor image quality and tiny layout severely restrict rearward visibility, making it dangerous to spot pedestrians, obstacles, or small children when backing out of driveways. …”
NHTSA complaint #11769045, filed 2026-10“The contact owns a 2026 Toyota Sienna Hybrid. The contact stated that while the vehicle was occupied and parked in the driveway, the sunroof exploded outward without impact. The sunroof shade was closed during the failure. There was no reported injury. The vehicle was taken to a dealer, where the contact was informed that a similar failure was previously reported; however, the failure was not covered under warranty. The vehicle was not diagnosed or repaired. The dealer referred the contact to the manufacturer, who then referred the contact to the sunroof glass manufacturer for assistance. …”
