“The passenger headlight will go out on my 2011 Mazda 6 even though the bulb and fuses are still intact and working properly. It worked when I parked my car and turned them off manually and then did not come back on the next time I started my car and turned the lights on. It is definitely a flaw from Mazda as numerous people have had this same problem. It is not only the 2011 models, search for Mazda headlight issue and a variety of the Mazda 6's are having this same issue.”

NHTSA complaint #10691766, filed 2015-03, 52,145 miles

“The bulbs on my vehicle seem to blow out within a month or 2. We tried different bulbs and it still happens. We are spending tons of money replacing headlights and the car is only driven a couple miles a day.”

NHTSA complaint #10715543, filed 2015-05, 32,075 miles

“When you put the vehicle in park and press the engine button the car does not turn off, you have to move the shifter back and forth several time to get the car to power down.”

NHTSA complaint #11318357, filed 2020-03, 68,000 miles

“The vehicle's car alarm goes off at random times. Generally it seems to happen only in warmer weather and/or only if the key fob is within a certain distance. So far changing batteries in both the car and the key fob have not changed anything. There appears to be flaw in the door mechanisms that make the car think someone is breaking in due to loss of contact when the car has been locked. This only happens when the car is parked and locked by any method. It does not happen if the vehicle is let unlocked or in cooler temperatures. This issue has been ongoing for the past three years.”

NHTSA complaint #11012164, filed 2017-08, 80,000 miles

“Headlight on both sides go out often. After having this issue far too many times, I looked online hoping to find a solution and discovered multiple Mazda owners are having same problem. Its a malfunction in the Mazda that needs to be fixed by Mazda this happened ever since I bought the car in 2011 I had to replace the headlight bulbs twice a year at least .”
