“The contact owned a 2005 Mitsubishi Galant. While driving approximately 40 MPH, the contact's vehicle crashed into the rear of another vehicle. The air bags failed to deploy. A police report was filed. The contact sustained injuries to the chest, back, and neck. Medical attention was required. The vehicle was destroyed and towed to an independent towing facility. The VIN and failure mileage were unknown.”
