Example complaints

Brakes: “Active Technical Service Bulletin that was not disclosed by Toyota or my dealer. The brakes make a piercing metal on metal noise when placed in reverse. I've been told this is not a safety issue, there will be no longer term impacts, and it is safe to drive. The issue however is that Toyota is limiting distribution of available repair parts, so I am on a 5-6 month wait list with my dealer. Bulletin is: T-SB-0046-25 Rev2”

NHTSA complaint #11735622, filed 2026-05

Electrical system: “My vehicle started when I left work and about 1/4 mile down the road every dash light came on and said Check Engine Reduced Engine power, Secondary collision Brake System malfunction, Hybrid system malfuncion, System malfunction, Drive start control malfunction. Then when I got out there was gasoline pouring from the bottom of my car running down the parking lot. It was a very dangerous situation. I was very afraid because someone in the parking lot was smoking. I called the Toyota towing service and they came and towed it to Doc's Toyota. That's where I purchased the car.”

NHTSA complaint #11770720, filed 2026-10

Engine: “1. Brake System Failure-Recurring dashboard warnings including “Brake Power Low” and “Secondary Collision Brake System Malfunction.” Warnings have appeared multiple times, including while towing a travel trailer. The vehicle was diagnosed with related fault codes (including DTC C138071 – Stop Lamp Relay). The problem returned after a dealer service visit. I do not consider the vehicle safe to drive or tow while these warnings are active. 2.Issue 2 – Engine (Metal Debris / Elevated Iron) Metal filings and shavings have been found repeatedly on the magnetic oil drain plug. …”

NHTSA complaint #11754489, filed 2026-08

Forward collision avoidance: “One Driver + one Passenger. Pulled into parking spot very slowly. Vehicle was moving about 1 mph, ready to pull to a stop, still in Drive. Vehicle suddenly surged forward with incredible energy and noise. Vehicle sped off parking lot and ascended steep ~6-foot berm of forest soil (no boulders). Driver applied brake, but it would not work. Driver later reported many red light warning indicators on the dash. Vehicle ran over saplings and on the passenger side sideswiped tree branches and tree trunk. Vehicle stopped suddenly. Incident lasted ~2-3 seconds. …”
