Example complaints

Selected from NHTSA records; personal details are removed. They are consumer reports, not findings.

“Road debris damaged AC condenser that cools batteries of RAV4 Prime. A quick google search yields many other cases over multiple model years highlighting a design failure on the part of Toyota. Not only is the repair costly, if not quickly remedied could lead to damaged hybrid batteries and possibly even fire.”

NHTSA complaint #11709171, filed 2026-01

“The contact owns a 2024 Toyota Rav4 Prime. The contact stated while driving at approximately 45 MPH, there was an abnormal explosion coming from the roof of the vehicle. The contact then became aware that the panoramic sunroof had shattered without impact. An unknown local dealer was notified of the failure and referred the contact to the Insurance Company for assistance. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 500.”

NHTSA complaint #11627412, filed 2024-11, 500 miles

“Windshield cracked by itself while pulling out from home garage. No visible signs of hit by foreign object. Cannot feel the crack with finger tips from inside or outside. Crack seems started from defroster.”

NHTSA complaint #11634774, filed 2025-01

“Panoramic moon roof exploded while driving with no impact to the glass. The explosion sounded like a gun shot inside the car. This sudden explosive noise put the driver, and all other vehicles on the road, in danger due to the panicked reflex of the driver. Upon inspection, it's evident that the glass shattered along all 4 edges rather than from a single point of impact. This would indicate an issue with the glass, the installation, or both. Vehicle was taken to dealer and customer was told it would not be covered under warranty.”
