“Frontal collision with another vehicle, impact was below 45 MPH. Seat belt failed to hold, and driver sustained severe injuries. Both the driver and passenger seat belts failed and the driver and the passenger suffered sever injuries .the driver of v1 was injured and reportedly died about 2 hours later.”
