Component/System failure & Inspection Availability The primary failure was the Supplemental Restraint System (SRS); the driver’s frontal airbag failed to deploy during a high-energy frontal impact. Additionally, Advanced Driver Assistance Systems (ADAS)—specifically Forward Collision Warning (FCW) and Lane Departure Warning (LDW)—failed to provide alerts or initiate emergency braking prior to impact. 2.
