“My Tesla displays a mandatory FSD (Supervised) disengagement feedback survey on the center touchscreen while the vehicle is in motion. Whenever I disengage FSD and resume manual control, the vehicle automatically asks me to select a reason for the disengagement. …”
NHTSA complaint #11767930, filed 2026-09“I was using actual smart summon to get my vehicle out of my garage. While standing behind the vehicle, it indicated that the car was in drive with no reversing lights on. The vehicle then rapidly lurches backwards, striking me and a freezer. After reviewing the dashcam footage the vehicle stated it was in drive and never once activated its reverse lights, even when they was objects and a human behind the car, still reversed at a high rate of speed.”
NHTSA complaint #11739971, filed 2026-05“I continue to experience phantom forward collision avoidance warnings. Tesla has looked at this several times, even once charging me $60 to clean the cameras (should be under warranty in my opinion). Latest response to problem: known issue, no known hardware cause. In other words live with it. They even suggested buying FSD package at $1200/yr as a solution! One more than one occasion the TACC and Autosteer disengaged with traffic behind me. Very scary.”
NHTSA complaint #11720733, filed 2026-02“Several times while using Traffic Aware Cruise Control, the system will suddenly brake severely for apparently reason. At these times, I was on a lightly traveled Interstate with no vehicles ahead of me. Each time, it was during daylight hours and it was sunny. This has also occurred while using Autopilot with Autosteer and with Full Self Driving. Once, there was a vehicle close behind me, so this sudden braking can be dangerous.”
NHTSA complaint #11606236, filed 2024-08“My wife and I recently purchased a new 2024 model 3 Tesla with the Autopilot and Full Self-Driving feature. We chose this vehicle because my wife has a long daily commute and we wanted to make the drive easier for her. We test drove a similar model in early August. When we were testing the Full Self-Driving feature, the car failed to safely exit the freeway three times in our short test drive, causing my wife to have to take over. …”
