“My husband was driving my car, we arrived at the parking lot and as he was pulling into a parking space and coming to a stop, the car suddenly accelerated at high speed, went over the cement bumper for the space, over the one for the space in front and hit a parked saturn in front of us. He is certain that he was braking at the time. We took the car in to Tesla who ran a diagnostic. …”

NHTSA complaint #11066047, filed 2018-01, 4,960 miles

“I was approaching a stop sign with my foot lifted above both pedals (brake and accelerator) when my car accelerated forward. I had always thought that most of these unintended accelerations were due to a driver hitting the accelerator pedal instead of the brake pedal so I noted that my foot was not resting or pressing on either pedal. I put it down on the brake pedal and the car stopped, but this should never have happened. I had no cruise control or steering aid activated. I also have 'creep mode' turned off.”

NHTSA complaint #11300542, filed 2020-01, 63,440 miles

“Driving on a 2 lane highway using autopilot the car accelerated and pulled left, ended up in the opposite lane and almost off road over a cliff. This is not the first incident I had with my Tesla. On several occasions, while using cruise control, it suddenly braked in the middle of the highway, while the immediate collision alarm went off. There were no vehicles in front of mine and no immediate risk for collision. Tesla needs to improve the autopilot features as there are significant safety concerns.”

NHTSA complaint #11258648, filed 2019-09, 64,000 miles

“When stationary or in motion, both on city streets and highways, and even turning the following is true. The center infotainment stack becomes completely frozen and unusable. Following this, it will automatically restart, however during this entire time, vital safety features controlled through the center infotainment stack are unavailable, including back up camera functionality. I have applied for the warranty program Tesla has for replacing the mcu1, however, each time they inform me my car does not meet the requirement without elaborating any further. …”

NHTSA complaint #11390231, filed 2021-01, 89,000 miles

“I was stopped at a stop light on my way to work around 8am on a very crowded city street. As the light turned green, I slowly pressed on the gas to move forward and the car took off at top speed. I hit the brake but the car did not respond - it did not slow down or stop, nor did any alarm, either visual or auditory, go off inside the car. To hit the car in front of me and then had to swerve to avoid hitting dozens of cars in my path. I grazed past a lamp post, another car and finally crashed into a tree.”
