“My drivers side front headlight issue first began about 50k miles (oct. 2012). The headlight went out and I went to replace the bulb but it wouldn't turn on. My mechanic fixed the brakes around the same time and took around the block, went over a bump and the bulb came back on. I've had issue ever since. Usually I can give it a little tap and it will come back on. Not this time! Ive replaced the bulb twice since 2013, it's at 80,555 miles now and it won't come back on at all! …”

NHTSA complaint #10731854, filed 2015-07, 50,000 miles

“I was driving with my 7 year old in my 2005 Mazda 6 when I noticed the car was accelerating on its own just a little bit. I didn't think much of it at the time. About 2 minutes later the rpms went up and down 3 times( 1000 rpms each time). Immediately after that, my brakes stopped working while I was going about 35 MPH. I had the brake pedal floored and it barely stopped in time. I managed to stop with my emergency brake and put on my hazard lights for a few minutes while the car was still running. …”

NHTSA complaint #10614668, filed 2014-07, 75,200 miles

“I was driving home from work today. I was driving on a road not far from my home and going about 40 MPH. My foot was not on the gas but the car began to suddenly accelerate. It suddenly accelerated to over 50 MPH. I began to brake, but it felt like I was trying to brake at the same time someone was pressing hard on the gas pedal. I put the car in neutral and pressed the brake as hard as I could. It was a definite hard push on the brake -- as if I had lost power brakes. There were cars ahead of me stopped in the road because of an accident. I managed to stop -- but barely. …”

NHTSA complaint #10542187, filed 2013-09, 165,000 miles

“The car unexpectedly accelerated with my foot of the gas pedal. I was driving around a curb at about 20 MPH and all of sudden it increased to 40 mphs, and the RPM needle jumped up and down, and finally it got stuck at 3rps. I was on a flat surface and the vehicle continued to accelerate and make loud engine sounds. I tired to brake but the car continued to accelerate, I kept driving until I was able to let other cars pass, and pull over and put it in neutral. In neutral the car still accelerated, and made noises, that I then put it in parked. …”

NHTSA complaint #10428127, filed 2011-10, 78,900 miles

“Takata recall. The contact owns a 2005 Mazda 6. While driving 35 MPH, the vehicle independently accelerated without warning. The brake pedal was depressed to the floor, but the vehicle failed to respond. The contact mentioned that the vehicle accelerated up to 50 MPH and stopped after the emergency brake was engaged and the ignition key was removed. The vehicle was not diagnosed or repaired. …”
