As I approached the intersection I saw the car come forward and then come to a complete stop before the intersection I assumed to look both ways. …”

The contact's spouse stated that while attempting to make a turn, the brake pedal was depressed and the accelerator pedal was inadvertently applied. The contact's spouse was unaware whether any warning lights were illuminated due to the failure. The contact stated that the vehicle unexpectedly accelerated, causing the vehicle to crash into a sidewalk and then into a ditch. No injuries were sustained at the time of the crash, and no medical attention was sought immediately following the incident. However, the contact later suffered neck and knee injuries. …”
