Selected from NHTSA records; personal details are removed. They are consumer reports, not findings.

“Headlights are so frosted/yellowed you can.t see at night.the lights on this vehicle were not that good when they were new. But now that the plastic has broken down from exposure to the environment, the lights are useless. My older cars had headlights made of glass and did not seem to have this problem.”

NHTSA complaint #10177292, filed 2006-12, 125,000 miles

“Plastic covers of 1997 Plymouth Voyager minivan headlamps are losing transparency. Minute cracks appearing on the surface of the headlamps are making them less clear and thus decreasing the brightness of the headlamps at night. Noticed initially a year ago , slowly getting worse.”

NHTSA complaint #10043286, filed 2003-10, 150,000 miles

“I have seen all of the other vehicles be recalled due to inadequte lighting by the head lights but I still do not see the crysler minivans being recalled for this problem. They are the worst. Please check on this and force crysler to recall them.”

NHTSA complaint #763748, filed 2002-07

“The contact stated that both front headlight covers are cloudy and discolored, which makes it difficult to see the road ahead. The manufacturer was contacted on 07/24/06 who determined that the whole headlight assembly needed to be replaced at cost.”

NHTSA complaint #10163329, filed 2006-07, 28,000 miles

“Consumer complained about the headlights. Both headlights have glazed over, almost opaque, resulting in extremely reduced visibility during the night. Thus, obstructing consumer's view while driving. Dealership indicated nothing they could do about it, only to replace each headlight unit. Each headlight unit would cost $263.00 at consumer's expense.”
