NHTSA complaint #11019998, filed 2017-08, 105,600 miles

“I was driving on a dirt road around 25-30 MPH when my drivers side curtain shield airbag (csa) deployed unexpectedly. Previous to deploying, the seatbelt had suddenly locked into place. I leaned back to get the seatbelt to release, and as I tried to lean forward to see if it had, the airbag suddenly and unexpectedly deployed. I stopped the vehicle and looked around the car for damage from a collision and observed none. I also did not feel or sense a collision happening. I believed this issue may be related to a Toyota airbag recall, prompting me to contact them. …”

NHTSA complaint #11387223, filed 2021-01, 138,000 miles

“On july 20, 2010 I was driving my 2007 Toyota RAV4 when I was hit by another car at a high rate of speed. My seat belt failed to restrain me and I hit my head, requiring stitches and a cat scan. My car is currently at a collision center waiting to be evaluated. I have not previously had any incidents where my seat belt had a reason to restrain me, so I do not know if this is an isolated incident or a possible seat belt defect.”

NHTSA complaint #10344902, filed 2010-07, 66,650 miles

“Vehicle was being operated at low speed (15-20 MPH) on residential street approaching a stop sign. Under braking, driver's and passenger's side curtain airbags deployed and driver's seat belt tensioner fired.”

NHTSA complaint #10895911, filed 2016-08, 128,000 miles

“I bought this 2007 rav4 late last year and JUST discovered, when trying to lock in a car seat (my first time with back seat passengers) that two of the rear seat belt buckles do not work. Both the rear passenger and the middle seat belt buckles do not allow the seat belts to lock. Therefore I can only have one passenger in the back seat.”
