Example complaints

Brakes: “I was driving my car on the highway. The car automatically deployed the emergency braking system with no objects in front of the car. The car automatically braked then flashed the brake warning indicator. Car mad itself come to a complete stop. I began researching and found that others have had issues with “phantom braking” when the car has collision avoidance technology. This needs to be seriously investigated as it is a safety hazard.”

NHTSA complaint #11514744, filed 2023-03

Other or unknown: “The rearview mirror fell off when I lightly knocked with my sunshade. The Mazda dealership reattached the mirror when the car was in for an oil change. But then a year later, the same thing happened. I took my car to get an oil change at our regular mechanic and they re-attached the mirror, but then it fell off in my hand a week later when I tried to adjust the mirror. I took the vehicle back to the dealership to get the mirror reattached and they quoted me $105 to reattach the mirror. …”

NHTSA complaint #11767989, filed 2026-09

Forward collision avoidance: “While driving down the road a vehicle in front of me braked to stop for a bus loading kids, as I stepped on the brakes nothing happened and the vehicle surged forward. After the accident I viewed the vehicles onboard system status and it had logged several errors including errors to the activsense driver assist control. I used the vehicle’s emergency assist button and contacted 911, and reported the incident to ride share and contact my spouse. Police report lists the wrong time and vehicle but does have the correct vin number”

NHTSA complaint #11724381, filed 2026-03

Electrical system: “The contact owns a 2023 Mazda. The contact stated that the vehicle was braking randomly while driving. There was an unknown malfunction code that flashed quickly. The steering was stiff and failed to turn. The steering wheel adjusted and guided the vehicle into the wrong lane. The RPM was sporadically reeving. The RPM increased, and the vehicle stalled without acceleration. There was smoke emerging from the hood. There was oil leaking. The vehicle failed to turn off and on. The fuel gauge and infotainment center were not working properly. …”
