“I had a front end collision with a guardrail at about 45 to 50 miles an hour the airbags did not deploy. Ram truck is coming to look at it to see why they didn't deploy. I didn't know where else to turn to that's why I am contacting you and letting you know what happened. Nobody was seriously injured I was on route us 35 heading west in beavercreek ohio.”
NHTSA complaint #11114545, filed 2018-07, 58,000 miles“Takata inflator failed when in a front end crash due to a drunk driver at 45mph. Drunk driver hit car in front of me and I swerved to the left and t-boned drunk driver on passenger side door causing him to spin around and slam into driver side of bed. With this kind of impact air bags should have deployed and fuel cut off should have engaged, but nothing happened.”

NHTSA complaint #11006792, filed 2017-07, 55,850 miles“After direct frontal impact to rear of stationary semi tractor trailor on an interstate in heavy traffic the driver struck steering wheel with seat belt on resulting in moderate injury requiring transport to emergency room for evaluation but not hospitalization. Speed estimated at about 30 MPH. Extensive damage to front end of vehicle. Airbags did not deploy. See pictures.”
NHTSA complaint #10967137, filed 2017-03, 61,000 miles“Involved in a mva during a snow storm. Responding to a fire call driving down hill when vehicle infront stopped in middle of the road. Tapped on brakes and truck started to slide. Truck lost control due to slippery road conditions .truck went through a tree and telephone pole and my seat belts never locked and air bags did not deploy. Trucks front frame was wrinkled and bumper snapped in half. While stepping on the brakes seat belts never locked.”
