“I was changing the left front tire because the tire had a nail in it. I used the tire jack from the vehicle. The vehicle was raised and the tire was removed and the jack collapsed. The vehicle was perfectly level in a garage with a cement floor. The vehicle was in park. I did not set the emergency brake or block the tires. Nothing was touching or rocking the vehicle at the time of the incident. Looking at the jack afterwards, the screw bolt was bent and the upper arms were bent.”

The contact stated while driving 60 MPH, a child's finger became stuck in the headphones that were equipped for the factory dvd player. The contact mentioned that there was a defect in the headphones. The vehicle was not taken to a dealer. The manufacturer was made aware of the failure. The failure and current mileage was 8,000.
